Back to Home program

Transitional housing is sometimes necessary for patients who have completed the acute care phase of their treatment but aren't able to transfer back to their homes based on the kind of care these Alternate Levels of Care (ALC) clients still require. Riverside Health Care's Back to Home for ALC program has a number of beds with 24/7 on-site support for ALC patients transitioning back to independent living.

Our services include case management, enhancing activities of daily living and building social and life skills. The program is fully accessible for those with physical challenges.

Referrals can be made by our Health Systems Navigator or Patient Flow Coordinator.
