Position Summary

The Supervisor of Community Support Service will be responsible for the supervision and leadership of all programming, with direct oversight of the Community Support Services Coordinators. The Supervisor will be required to ensure benchmarks for quality care and productivity are met. The position assists the Community Services Director in the planning, organizing and delivery of all community support services. The supervisor’s position monitors both clients and staff to ensure appropriate services are delivered. The position has responsibility for performance management of staff under their direction including performance appraisal, training and development, and if required, discipline.

Qualifications Required

  • · Certificate of Competence from the College of Nurses of Ontario (RPN), OR Two-year post-secondary diploma in business administration, healthcare administration, or an equivalent combination of education and work experience as deemed appropriate by Riverside Healthcare Facilities Inc.
  • · Minimum 3 years related experience in a healthcare setting, including experience in community environments.
  • · Experience with supervision of staff, scheduling of staff, day to day oversight, performance management, and labour relations preferred.
  • · Knowledge of budgeting, and financial management preferred
  • · Demonstrated knowledge and understanding of Home and Community Care Support Services locally, regionally and provincially.
  • · High degree of proficiency using office software applications, including word processing, spreadsheet and email software.
  • o Knowledge and understanding of software applications specific to Home and Community Care and Hospital sector (i.e. Health Partners Gateway and AlayaCare) is an asset.
  • · Ability to problem solve, exercise good judgement, build effective relationships, work productively in a team environment; and demonstrate tact and diplomacy.
  • · Demonstrated ability to function effectively in a fast-paced healthcare environment with excellent organizational and time management skills with the ability to multitask and prioritize workload in an office setting.
  • · Strong communication, customer service, and interpersonal skills.
  • · Demonstrates initiative / self-management, accountability and flexibility/adaptability.
  • · Ability to maintain confidentiality and has a strong understanding of applicable privacy legislation.
  • · Dependable, competent and continued ability to carry out responsibilities as assigned
  • · Understands and can ensure compliance with Occupational Health & Safety act.
  • · Must be physically capable of performing the essential duties of the job.
  • · Joint Health and Safety Committee certification is an asset and is required within one year of incumbent starting the position.
  • · Access to reliable transportation to work locations. Maintaining a valid Class ‘G’ Driver’s license is a condition of employment due to frequent travel requirements within the Rainy River District.

Specific Responsibilities:

Administrative / Human Resources

  1. In collaboration with Riverside Human Resources Department and in line with human resources policies and procedures, assists with the hiring, orientation, training, supervision, evaluation and discipline of the Community Support Services (CSS) staff.
  2. Oversees scheduling in accordance with appropriate collective agreements to ensure staffing is aligned to client and program needs. Will work with the Director of Community Services, and Human Resources to ensure appropriate staffing levels are in place.
  3. Accountable to ensure payroll submissions are provided on time and are accurate. Assist with payroll as needed.
  4. Handles private and confidential information relating to staff illnesses, disciplinary procedures, grievances and labour relation issues.
  5. Responsible to ensure all Occupational Health & Safety requirements are met regarding inspections, required meetings, and minute taking. Required to participate on the Joint Health and Safety Committee for CSS. Supervisor is required to obtain and maintain Joint Health and Safety Committee certification as per requirements defined by the Province of Ontario.

Clinical Practice Management

  1. Responsible for supporting the day-to-day operations of the CSS Programs; Assisted Living, Home Support, Adult Day, Contract Nursing an PSS, Hospital at Home, Meals on Wheels, Visiting Hospice and Hospital to Home.
  2. Coordinate and support safe and timely transitions for patients discharged from hospital to home and community-based services.
  3. Assists with the development and evaluation of policies and procedures ensuring compliance with all legislated standards and regulations.
  4. Monitor and maintain data on Riverside CSS waitlist details
  5. Conduct analysis and follow up of issues related to resident/client care, infection control, clinical quality and risk management programs.
  6. Provide support to residents/clients, caregivers, families by responding timely to request/concerns and problems.
  7. Liaison with community groups and agencies.

Fiscal/Resource Management

  1. Assist the Director of Community Services in the capital equipment budget.
  2. Accounts for petty cash fund and other monies received by the clients related to accounts receivables and general deposits
  3. Oversees and responsible for all CSS revenue collection and deposit.
  4. Maintain, prepare and distribute daily, weekly and monthly statistical information for OHaH contract services, Assisted Living, Hospital to Home, Meals on Wheels and Adult Day Programs.

Quality Improvement /Risk Management

  1. Participates in patient/client safety and risk management initiatives inclusive of investigating incidents with evaluation and trending of data.
  2. Performs duties in accordance with departmental policies and procedures, and recognized professional, quality, patient safety, environmental and infection control standards.

Other

  1.  Performs other such duties or responsibilities that may be delegated by the Director of Community Services.
