Rehabilitation Technician - Community Stroke Rehabilitation

REPORTS TO (Title):

Manager, Therapeutic Service

 The Riverside Community Stroke Rehabilitation (CRS) Rehabilitation Technician will support the therapists in providing person-centred and coordinated care for stroke rehabilitation. Delivered in a setting that best meets the needs of stroke patients who have experienced a stroke within the last year and/or have related chronic and complex diseases affecting their ability to live independently and successfully.

QUALIFICATIONS NECESSARY:

 

  •  Graduate of a PTA/OT/Rehabilitation Assistant Diploma program, equivalent and/or combination of experience may be considered.
  •  Excellent organizational and time management skills.
  •  Excellent multi-tasking skills and the ability to work well under pressure.
  •  Proficient computer skills.
  •  Experience with electronic documentation.
  •  Demonstrated strong written and verbal communication skills.
  •  Demonstrated ability to maintain strict confidentiality.
  •  Demonstrated ability to function effectively independently, within a team, with front line personnel, and management
  •  Must be able to follow verbal and written orders.
  •  Physically capable of frequent bending, stretching, and lifting and of assisting heavy-needs patients with bed mobility, transfers, ambulation, and exercise program.
  •  Class G drivers license, access to reliable vehicle with and the ability to travel to various locations within the catchment area.

 

BASIC RESPONSIBILITIES:

 

  1.  Functioning as part of the interdisciplinary health care team to assist in meeting basic needs of patients in accordance with the policies, procedures, and standards of Riverside health Care Facilities.
  2.  Carry out all work assignments safely and be knowledgeable or in compliance with relevant regulations, policies, and procedures.
  3. Seek assistance and guidance when necessary, observing patient’s needs and self-limitations.
  4.  Support corporate rehab quality improvement initiatives.
  5.  Exercise behaviour consistent with Riverside Health Care’s Code of Conduct.
  6.  Demonstrates Riverside Health Care’s Core Values: Progressive, Integrity, Caring and Accountable.
  7.  Demonstrate a tactful, empathetic, and caring patient approach, and encourage independence.
  8.  Carry out treatment plans/document as designed by the therapist.
  9.  Monitor any changes to a patient’s health status and report to the appropriate therapist.
  10.  Booking appointments, making phone calls, connecting patients with community resources.

 

DUTIES AND RESPONSIBILITIES:

1.  Delivers patient care according to the philosophy, goals and objectives of the Rehabilitation Services department. 

  •       Assessment: Consults with therapist to obtain patient history, ambulation, and transfer needs. 
  •       Planning: Assist the therapist to develop a plan of care and communicate any pertinent information accurately and promptly to the therapist.
  •       Implementation: Carry’s out treatment plan as designed by the therapist. 
  •       Evaluation: Continuously evaluates the outcomes of the therapy sessions and communicates with the therapist
2. Participates in departmental and related committees/meeting when required. 
3. Participates in educational activities and programs for patients. 
4. Reports any untoward/unusual incidents accurately and promptly to the therapist. 
5. Participates in the department’s quality management program.
6. Other such duties or responsibilities as may be delegated.

REPORTING:

  • This position reports to the Manager, Therapeutic Services.

POSITION AVAILABLE:

  •  This Casual position will commence: As soon as possible.
