The Community Support Services department has an opening for Part Time Personal Support Workers at the Fort Frances location. The successful candidate must be able to meet the normal requirements of the job and possess the following:

QUALIFICATIONS NECESSARY:

  • Personal Support Worker diploma / certificate preferred.
  • Completion of secondary school diploma, or equivalency, or the equivalent, knowledge, skills, and experience.
  • Valid CPR certification.
  • Valid Driver’s License, Class G, and a safe driving record.
  • Effective communication skills, both verbal and written.
  • Understands and maintains confidentiality.
  • Experience working with the elderly in a community setting.
  • Ability to assess and respond to client needs and set priorities.
  • Ability to work independently, as well as in a team setting.
  • Genuine interest and understanding for the care and need of the elderly.
  • Knowledge of the resources for seniors available in the community.
  • Dependable with an excellent attendance record.
  • Physically capable of performing the essential duties of the job.

BASIC RESPONSIBILITIES:

Under the direction of the Manager of Community Support Services (CSS), the Personal Support Worker (PSW) will provide a range of in-home services and support to eligible, high-risk seniors. A focus is placed on providing client centered, client directed and community-based care. As directed by the service plan, PSW will support clients by assisting them to achieve the highest degree of health and independence possible through the promotion and support of activities for daily living.

SPECIFIC DUTIES:

  1. Assist and support clients with activities for daily living as determined by their service plan. Document all care in the CSS Electronic Medical Report (EMR).
  2. Assist and support clients to access community services and resources that relate to their goals and needs. Assist in coordination of services including transportation services, booking, and confirming health and wellness appointments, pickup of medications, ensuring food security, etc.
  3. Assist clients with range of motion regiments found in the care plan as directed by Occupational Therapists and Physiotherapists and promote community-based exercise programs tailored to seniors, as instructed.
  4. Provide well-being checks and services that aid in preventing the negative effects of social isolation to ensure client health and safety.
  5. Support clients with medication compliance. Monitor and document the completion of required medication reporting such as medication changes, concerns, refusals, and ensure that medication safeguards are in place.
  6. Prepare nutritionally appropriate meals and snacks, and/or provide meals appropriate to the client’s preferences and needs as set out in the care plan.
  7. Assist in the proper utilization and care of all adaptive aid equipment required by the client.
  8. Ensure cleaning tasks are completed to promote the health and safety of the client.
  9. As required, accompany clients to scheduled appointments, assist with errands, and/or provide support for clients to attend community-based activities that promote social well-being.
  10. 10. Report all changes in the client’s overall health to the CSS Coordinator as required, and document in the EMR.
  11. 11. Provide personal care to the client by assisting with activities of daily living including bathing, eating, dressing, personal hygiene, grooming, toileting, and ambulation as set out in the care plan.
  12. 12. Provide routine health care that may be outside of the care plan, such as skin care, health monitoring, and other care tasks as directed by the CSS Coordinator or delegate.
  13. 13. Shift times and duration vary between 07:30 and 22:00, 7 days a week, including holidays.
  14. 14. Report any substantial findings and/or changes in client behavior to the CSS Coordinator and document in the EMR.
  15. 15. Carry out work assignments safely and always comply with the Occupational Health and Safety Act.
  16. 16. Provide replacement coverage for staff according to call-in procedure and seniority.
  17. 17. Perform other duties as assigned
