Community Service Management Program

Program Objectives

• To provide a community-based worker who will offer support to people who experience psychiatric symptoms that impair their daily functioning.
• To advocate and to facilitate for continuous improvement of those services which exist to meet the needs of people exhibiting psychiatric symptoms.
• To support local residents in gradual reintegration into the community from psychiatric facilities.

The Case Management Worker is available to assist/support consumers in accessing appropriate housing, education, employment, health services, social and informal supports to improve overall health status.

Eligibility Requirements

Individuals 18 years of age or over who have exhibited psychiatric symptoms and wish to participate in the program.

Referral Method

Referrals can be made by family, self, physicians, friends or other agencies.
